Does TRICARE Cover Therapy?

Key Takeaways

  • TRICARE covers therapy when it is medically necessary. The benefit includes office-based outpatient psychotherapy in individual, family, and group formats, along with psychological testing, medication, and other services. Referral requirements, provider options, and costs may vary depending on your plan.
  • Referral rules depend on your plan and your status. Active duty members generally need a referral and pre-authorization to see a civilian provider, while most other beneficiaries can schedule outpatient visits directly. Checking your plan’s rules before you book can save you a denied claim.
  • Free counseling exists outside TRICARE entirely. Military OneSource provides free, short-term, confidential non-medical counseling to service members and their families, in person or by phone, video, or chat. You can contact Military OneSource to learn more about available counseling options and how to get started.

If you are covered by TRICARE and thinking about starting therapy, the good news is that TRICARE covers mental health services for eligible service members, retirees, and family members.

However, finding a therapist and understanding your coverage can sometimes feel confusing. Referral requirements, provider options, and out-of-pocket costs can vary depending on which plan you are enrolled in and military status.

This guide breaks down what you need to know so you can feel more confident taking the next steps toward getting support.

What Mental Health Care Does TRICARE Cover?

TRICARE covers a variety of services for mental health and substance use disorders. Care is available at military hospitals and clinics and from TRICARE-authorized civilian providers.

Coverage generally rests on medical necessity, which means a provider documents that the care is needed to treat a mental health condition. TRICARE covers several types of outpatient mental health services, including:

  • Individual psychotherapy. Sessions are covered up to 60 minutes, and individual crisis sessions may run longer.
  • Family or conjoint sessions. Therapy designed to treat the whole family, covered up to 90 minutes for regular sessions.
  • Group sessions. Therapy with several patients treated together, covered up to 90 minutes.
  • Psychoanalysis. Covered when the provider holds the right certification and the treatment is pre-authorized.
  • Psychological testing and assessment. Covered when it is medically or psychologically necessary to diagnose and plan treatment.
  • Collateral visits. Visits between your provider and a family member or other responsible person, covered when needed.
  • Psychotropic medication. Covered when prescribed by an authorized provider as part of a treatment plan.

Marriage counseling is the exception people run into most. TRICARE covers it only when it is necessary to treat a diagnosed mental condition. Counseling focused solely on relationship difficulties typically isn’t covered.

Do I Need a Referral to See a Therapist?

This is where plans differ most, and it is the question most likely to trip up a first appointment.

  • Active duty and activated Guard or Reserve. You should get care at a military hospital or clinic when possible. Seeing a TRICARE-authorized civilian provider generally requires a referral and pre-authorization coordinated by your primary care manager.
  • TRICARE Prime. You generally do not need a referral for outpatient visits with a network psychiatrist or psychologist, except for psychoanalysis and outpatient substance use disorder therapy. Going outside the network without a referral can trigger point-of-service fees.
  • TRICARE Select and similar plans. You can generally schedule with any TRICARE-authorized provider without a referral, with the same two exceptions.
  • US Family Health Plan. You can search your plan’s directory and generally do not need a referral for outpatient visits.
  • TRICARE For Life. Medicare’s rules generally apply first, until those benefits are exhausted.

Inpatient mental health care generally requires pre-authorization across plans. Emergencies are the exception. In a mental health emergency you do not need a referral or pre-authorization, though an admission still has to be reported to your regional contractor quickly.

Provider type matters too. For example, under TRICARE Prime you can see a supervised mental health counselor or pastoral counselor, though a physician must supervise that care. However, certified mental health counselors do not require physician referral or supervision.

What Does TRICARE Not Cover?

A few common requests fall outside the benefit, and knowing them ahead of time avoids an unwelcome bill.

  • Marriage counseling that is not tied to treating a diagnosed mental disorder
  • Psychological testing for academic placement, job placement, or child custody disputes
  • General screening when there are no specific symptoms
  • Testing to determine whether someone has a learning disorder or learning disability

Therapies such as art, music, and dance are covered only as part of a treatment plan at certain facilities, but they are not covered as standalone outpatient services.

How Much Does Therapy Cost With TRICARE?

Costs depend on which plan you are enrolled in, your sponsor’s status, and whether the provider is in the network. TRICARE publishes current cost-share amounts and updates them from year to year, so your plan’s own cost tools are a more reliable place to check than a figure you read somewhere.

One cost trap comes up often. For example, if you have TRICARE Prime and you see a non-network provider, or a network provider in another region, without a referral, you may end up paying point-of-service fees. Confirming your provider’s network status and any referral requirements before your first appointment can help you avoid unexpected costs.

Are There Free Options Besides TRICARE?

Yes, and they sit outside the TRICARE benefit entirely, which means no referral and no claim.

Military OneSource counselors hold a master’s or doctoral degree and a license to practice independently, and sessions can happen in person, over the phone, or by secure video or chat. Military OneSource counseling is confidential and generally does not affect a service member’s or spouse’s career or security clearance. However, confidentiality has certain exceptions, including safety concerns and legally required reporting.

The Military and Family Life Counseling Program provides free, confidential counseling to service members, families, and survivors on or near installations, covering short-term concerns such as everyday stress, deployment and reintegration, parenting, and grief.

Both programs are built for short-term, non-medical support rather than diagnosing or treating a mental health condition. When something calls for clinical care, they generally point you toward TRICARE or your nearest military treatment facility.

How Do I Find a Therapist Who Takes TRICARE?

Start with your region’s provider directory. Regional contractors maintain listings for both network and non-network providers, and military hospitals and clinics are worth checking as well, particularly for active duty members.

Finding a therapist who accepts your insurance is important, but finding someone you feel comfortable with matters, too. Rapport and trust are essential, and a short preliminary conversation tells you a lot. Our guide to the types of therapists explains what the different licenses mean in practice.

We created Mental Health Match to make the search part easier. You answer a few questions about what you are dealing with and what you want in a therapist, and we introduce you to therapists who fit. It is free to use, and insurance is one of the things you can tell us about. However, make sure to confirm TRICARE participation with the therapist’s office directly before your first appointment, since billing runs through the practice rather than through us.
